The subways get you to virtually any place you want to go and I never felt unsafe on the subway - although I never took it after about 8:00pm. I've also been to the Washington D.C. area. The Metro is pretty good for connecting the various "burbs" to D.C. but it's not really for getting about in the city. I'm sure D.C., Baltimore, and the Beltway area all have good bus systems, but I've never really used them. You can make a similar statement about the San Francisco Bay Area. BART is really good for getting from city to city and then they have decent surface transit for getting about the city. At least I know the MUNI lines in San Francisco are pretty good. Sacramento has has a pretty good Regional Transit system - kind of a subway above ground - which runs through most parts of the city. It's been a while since I spent any real time in Sacramento, but I'm sure they also have good buses as well. All 4 of these cities/areas have very good doctors and hospitals and they all have ample shopping. For someone nearing retirement age, I would say the Sacramento area might be the better choice. Sacramento is a big city (although smaller than the others) and it has many suburbs that range from being extensions of the city to being small towns. With literally a 15-20 minute drive you can go from big city to small town. Plus the various communities within 60-90 miles of Sacramento offer a very wide variety of lifestyles as well as environments. I have lived within close proximity to Sacramento for virtually my entire life so I probably know it better than the other 3 cities. For djmaximus: I spent a week in Portland about 3½ years ago. I generally liked the city, but I didn't get the impression they had a robust public transportation system. But it I was looking for somewhere to relocate, Portland would be at the top of my list.
